What to Expect for Your Appointment

Please select New Patient Forms, you must complete both sets of forms under this tab. This includes, patient demographics, Consent and Treat, Medical Release, and Cancellation Form. You will also need to complete the Medical History forms, medication, allergy, and pharmacy information.
Our clinic usually receives your lab result within 5 business days, please understand once they are received your physician must review them before results can be given. We do order some labs that can take up to 14 days to complete.
In our clinic all patients are seen by the provider, we do have Nurse Practitioners they may see you if you are admitted to the hospital.
The most common test performed are Colonoscopy, EGD (Upper Endoscopy), ERCP (Endoscopic Retrograde Cholangiopancreatography) and EUS (Endoscopic Ultrasound). You can find additional procedures that we provider under the Procedure Tab.

At the time of your visit, we will discuss the potential cost for the services our provider will be performing. We will give you contact information for the other entities involved, to include the facility, anesthesia, and pathology so you may reach out to determine any additional cost to you.
As a specialist we will refill your medication at the discretion of the provider, some medication can be filled on a 90 day bases with additional refills. Some you must revisit every 6 months to obtain any additional refills.
